  1. Relevant referral is written and signed by GP or CDM Nurse

  2. Item Code 10953 (Exercise Physiology)

  3. The patient is then informed what Exercise Physiology is and about any. BODY Pilates & Exercise Physiology. Please link them any relevant videos from our website. Key information to mention:

    • Cost for Initial Assessments:

      • EPC initial assessment (60 mins) –

        • Senior AEP $180 upfront ($58 rebate from Medicare)

        • AEP $145 upfront ($58 rebate from Medicare)

    • Cost for Follow-up Appointments:

      • EPC  Follow-up Appointment (45mins)

        • Senior $145 upfront ($58 rebate from Medicare)

        • AEP – $135 upfront ($58 rebate from Medicare)

  4. Email Referral and relevant medical history to hello@anybody-studio.com.au

  5. We will provide consent to participate

  6. Patient can call the studio to book an appointment or we will contact them upon receipt of their referral
